Requesting an Appointment
To request an appointment, please use the button on the top right of your screen.
In addition, new patients and those who have not received treatment in the past year are required to complete our New Patient Registration, Medical History and HIPAA forms. If you would like to complete your paperwork before you come to the office, please use the following links to download the forms. Note: If you have downloaded all three of these documents, you should have five pages of paperwork to turn in. Also, you will still need to present your insurance card and proper form of identification upon check-in.
